Once completed please follow the returning instructions at the bottom of the form.
Please also note that your firm is required to complete the following document, unless previously completed.

2. Log into lloyds.com / access 'My account' / Click Member Modeller link in 'Tools and services'
3. Save the URL onto your favourites list and direct to the log-in screen from that link
